Over the Summer we are putting together information about all of our groups and programs. We would like to compile the most up to date information that we can pass on to the communications team, Rev. Sasha, visitors and others. It will also allow for leadership to be able to communicate with you as well. We are looking for ALL groups to fill this out, even if you have been a long term group or program, we would still like to to fill this out.
We will set up an email distribution list(s) for the leadership team, REALM group organization, listing on the Fellowship website, and other support.

There are a few sections to the form. (This makes it easier to fill out as some section appear based on previous answers.) I’ll put them here so you know what to expect:
1. General information about you and your group. This includes a Short and longer descriptions. For most use cases, the short (50-75 word) description is what will be used. There is no need to put contact information or dates any times in this field. There are other places for that. If you would like a longer description with more details, there is a field for that. People creating communications will use this at their discretion.
2. This is contact information. The point person is your point of contact. This can be a the chair or leader. Basically the primary contact. If this is shared, please put both people. Next please give us the leadership team with email adresses. This is who is planning events or meetings. This group will be forwarded all correspondence and have the opportunity to respond to those emails. They will also have access to any pertinent files. Make sure to include names and email addresses of these people. The last question creates a “volunteer distribution list.” This is a larger group of people who may need more detailed information. Some groups may not have this, so you can leave this blank.
As an example, the Faith Formation Committee is set up like this.
POINT PEOPLE: Committee Co-Chairs
LEADERSHIP TEAM: the 9 members of the committee
VOLUNTEER TEAM: All of the teachers. (This allows us to easily communicate and share certain files with the entire team.)
3. Administrative information is about what rooms you want/need, zoom support, budget requests (when we renew these groups, we will do so it will make sense then, and communication help.It should be pretty straight forward.
If you want to go back and edit it before you submit, use the back and next buttons at the bottom of the form. If you want to edit it after you submit it, a copy will be sent to your email (check your spam folder if you cant find it) Lastly, I only need on submission per group. However, if more come in, we’ll work that out.
